Register to have the N number assigned The forms to do this are FAA Form 8050-1 (a multi-sheet carbon form) Filling out 8050-1 and FAA Form 8050-88A "Affidavit of Ownership" blank form Docs/8050-88a.pdf As an Operating Fleet ELSA the "builder" should be identified as the Manufacturing Company. Sample Filling out 8050-88a Once you have your formal N number from the FAA on the 8050-3 "hard card", (see ELSA Cert. next page) you need to talk with the DAR and send the following: 1) A copy of the 8050-3 Hard Card (Note: This is absolutely critical and all following forms MUST match the Hard Card exactly!) It looks like this 8050-3 Looks like 2) Form 8130-6 filled out. This is an Airworthiness Application. It MUST match the Hard Card exactly! Make sure you include a complete list of installed equipment. If you use this sample Wt and Balance Spread sheet there is a place there for the list.
