Instructions for filling out the

8050-1 form

Application for Airworthiness

Note:  These directions will follow line by line on the 8050-1 (triplicate form)

Line 1:  Starts with N  DO NOT fill in this line unless you have an N number reserved.

Line 2: Aircraft Manufacturer & Model (example below)

            Aero Trike                    Safari  (on the same line, just leave a space in between)

Line 3: Aircraft serial number  (if you don’t have one pick one, it will be put on your data plate at inspection time.)

Line 4: Type of registration

             X box with Individual

Line 5: Name of appliant

(three boxes at bottom of form)

Remove back copy (pink) and last carbon.  DO NOT fold form, send 8050-1 form, fee of $5.00 to FAA, evidence of ownership (8050-88a form that is notorized)
