The applicant's coverage will begin after the following three criteria are met: receipt of this completed application, payment in full, and receipt and verification of required documentation. Once approved, a certificate and ID card will be issued for each pilot and emailed, faxed, or mailed to the applicant by First Flight Insurance Group, Inc.

Please include the following if applicable:

• Copy of ASC membership card for each pilot
• Private & commercial pilots must provide copy of 5 hrs logged pilot flight experience in type and class of insured aircraft
• Copy of pilot registration, certification or
• Copy of vehicle or aircraft registration license for each pilot
• This signed application
• Student Sport Pilots must provide copy of solo endorsement
• Check or credit card information
